New Patient Forms

If you are scheduled for a new patient appointment at our office, please click on our secure patient portal on the home page or my account link above to get started. Please follow the instructions to register and set-up your account. Please click on the health forms tab within the patient portal and fill out all 7 forms and remember to hit submit. This will take approximately 45 minutes. Please complete ALL forms 48 hours prior to your appointment or your appointment will need to be rescheduled. Medical Records Forms

If you would like to transfer your medical records to Optima Women's Healthcare, please download the medical records release form by clicking on the appropriate link below. Please fax the completed and signed form to 303.595.5390 or email to staff@optimawomenshealthcare.com...Please fill out one form per provider or healthcare facility from which records are needed. Thank you. We look forward to caring for you!

If you would like a copy of your medical records be sent to another provider, please download the medical records release form by clicking on the appropriate link below. Please fax the completed and signed form to 303.595.5390 or email to staff@optimawomenshealthcare.com. Please note the following:

We process and release medical records in the order of patient requests received.
It may take up to 30 days to transfer your records so please plan accordingly.
As a courtesy, there is no charge to transfer your records to another healthcare provider.
If you would like a personal copy of your records there is a charge. You can call our office to determine the cost and make pre-payment.
You can log on to our secure patient portal and access a clinical summary of each of your visits free of charge.
