An updated and signed financial policy and a credit card on file is required for all patients. Your credit card is stored securely and only used for incidentals such as missed copays, payment plan payments, and outstanding overdue balances. An alternative to providing your credit card on file, is to provide a deposit of $500 for incidentals and missed payments.
Prescriptons are written annually and require an in person visit once a year for refills. If you are requesting a new medication, you will need to schedule an appointment with Dr. Jerath. Any outstanding balance on your account must be paid prior to any prescription refills and you will be contacted by our office to make this payment. Please contact your pharmacy and ask them to send us an electronic prescription request and send us a message through our Spruce secure portal. We will do our best to expedite all prescription refills in a timely manner, but this process may take up to 3 business days. Prescription refills are not handled after hours.
Dr. Jerath takes call for her patients 24/7. If you have a medical emergency, please call 911. If you have an urgent after hours need, please call our office main number at 303.805.1807 and your call will be routed to Dr. Jerath's cell phone. Please leave a detailed message and she will call your back within 30 minutes. Please note: Dr. Jerath does not authorize or handle prescription refills after hours as these are handled by our Clinical Coordinator during office hours. Alternatively, you may send a message through our Spruce patient portal as Dr. Jerath checks these messages daily.
We are a busy practice and often have a waiting list of patients to be seen for appointments. We understand emergencies and unexpected conflicts happen. If you need to cancel your appointment, please kindly provide us at least 24 hours notice. We will always provide grace for inclement weather, illness, or personal emergencies. However, appointments cancelled without 24hrs notice (especially chronically) may incur a $50 charge.
Medical records are sent via fax only to another physician or provider as a courtesy at nor charge. Personal copies of medical records are sent digitally or via fax only and incur a $100 fee. We provide copies of recent paps, mammograms, and labs via our Spruce patient portal at no charge. Please fill out a medical records release form. All requests are processed in the order they are received. We will do our best to expedite your request, but it may take up to 30 days to process your request so please be patient and plan accordingly.
An annual well woman visit consists of reviewing your general medical history, a routine breast and pelvic exam with our without a Pap smear, pap smears, STI testing, and refills for contraception or hormones. We recommend this be done once a year and Rx refills are not provided without an up to date annual visit and exam. Most (not all) annual exams are covered under the Affordable Care Act (aka Obamacare) without a copay. However, if your visit is more complex and includes a more in depth discussion of any medical condition, further examination, and/or any add-on procedures then an E&M or procedure code will be submitted to your insurance and your visit may be subject to a copay, coinsurance, or deductible.

Any returned checks for insufficient funds will incur a $50 fee.
Dr. Jerath and her father are both immunocompromised and since we are in very close proximity during your appointment to minimize the risk of any respiratory infections, including covid, influenza, RSV, and other viral syndromes, we require everyone to wear a mask covering their nose and mouth throughout their time in our office. If you are unwilling to wear a mask, we are happy to transfer your medical records to another physician or healthcare provider. If we decide to make masks optional, we will update our policies.
